Description- Age, 21; height, 5 ft. 6 3/4 in.; weight, 140; hair, sandy; eyes, azure blue; complexion, light; build, medium; nativity, New York. Occupation, painter.

Tattoo in blue, red and green, bust of sailor boy, date 1908, letters P. H., at 5 in. above wrist, front. III. Small indented scar center forehead; small scar center forehead at hair line.
